Supports for older adults (aged 60 years and up), and adults with disabilities (aged 18 years and up) * helps people remain independent in their own homes for as long as possible * referral to other community services
Family Service Kent's Community & Home Assistance Program (CHAP) is dedicated to serving older adults and persons with disabilities. Our goal is to assist our clients maintain a healthy and independent lifestyle.
