103-bed long-term care home * residential care for 53 private rooms, and 50 basic rooms * 24-hour nursing and personal care * access to a doctor and other health professionals

bedding and furniture
laundry and housekeeping
meals and nutritional snacks
personal hygiene supplies
medical supplies and equipment (such as catheters, ostomy bags, or wound care supplies)
assistance with medication and activities of daily living (walking, bathing, dressing, etc.)
recreation and social programs (e.g. live entertainment, gardening, cooking, spiritual, montessori, etc.)

Meetings:

A multi-disciplinary team conference is held with the resident and/or substitute decision maker(s) approximately 6 weeks after the resident moves in to the home, to review his/her plan of care. A similar meeting will be scheduled annually thereafter.
