Ontario Stroke System

Organized stroke care, an integrated evidence-based approach to managing stroke, has been shown to reduce mortality and morbidity and improve functional ability and quality of life for stroke patients. Organized stroke care includes the entire continuum of care, from prevention to community re-engagement.

In May of 2007, the Provincial Stroke Steering Committee approved a new Strategic Plan for the Ontario Stroke System.

Vision: Fewer Strokes. Better Outcomes.

Mission: To continuously improve stroke prevention, care, recovery and re-integration.

At the same time, five strategic directions were identified:

Credible Advisor to Improve Stroke Prevention and Care Delivery: The Ontario Stroke System will be an expert resource, trusted by policy, planning and funding bodies for its information and recommendations.

Leadership and Coordination: The Ontario Stroke System will establish a sustainable infrastructure that provides leadership and coordination for stroke prevention and care delivery across the province.

Evaluation to Support Continuous Improvement: The Ontario Stroke System will implement a performance management system to evaluate progress in implementing best practices, as well as in achieving the Strategic Directions.

Innovation and Knowledge: The Ontario Stroke System will stimulate innovation and leverage knowledge across the continuum of stroke care.

Best Practices Across the Continuum of Stroke Care: The Ontario Stroke System will identify and implement best practices across the continuum of stroke care to reduce risks and improve health and quality of life for stroke survivors and their families.
