SWO STROKE STRATEGY STEERING COMMITTEE
Mission:
To facilitate, coordinate, implement and sustain the development of an integrated delivery system for best practice stroke care in Southwestern Ontario.
Vision:
To provide equitable access to best practice stroke care across the continuum, achieved through the establishment of a South Western Ontario coordinated stroke network.
Guiding Principles:
The steering committee will work collaboratively to improve and standardize the regional stroke care delivery system across the continuum of care. Within this context, the SWO Stroke Strategy Steering Committee will:
- Be person centred
- Enable care to be provided closer to home
- Build on best practices
- Use a capacity building approach in developing the regional stroke care network
- Share information to facilitate the development and implementation of district, regional and provincial stroke care plans
- Conduct evaluation and use as a basis for continuous improvement
- Advocate for resources that identify and/or address gaps in best practices
- Support and participate in research and knowledge transfer activities regionally and provincially
- Enhance academic linkages across the region
Regional Reviews
The Ontario Stroke Strategy (OSS) Strategic Plan (2007-2012)
identified the need to review the Regional Stroke Steering Committee
Terms of Reference and Regional Stroke Program Manager roles and
reporting relationships as a provincial priority. This review, led by
the Ontario Stroke Network (providing leadership and coordination
functions for the OSS) has been completed. Final recommendations are
provided in the documents below.
