Quick Reference Guide for Primary Care Providers
The Quick Reference Guide for Primary Care Providers was created to support health care providers manage stroke prevention in their clients.
The Southwestern Ontario Stroke Network (SWOSN) Quick Reference Guide is a two-page (double-sided), 8.5 x 11 document that includes medical targets, links to resources, and contact information for the Secondary Stroke Prevention Clinics in the region. It had been retired after the release of the national quick reference guide but was revived upon numerous requests of health professionals in the region.

The Heart and Stroke Foundation has created a national Taking Action in Stroke Prevention: A Quick Reference Response Guide
outlines the medial assessment and management individuals who experience a transient ischemic attack (TIA) or non-disabling stroke within 48 hours or 2 weeks of the onset of their stroke symptoms. Longer-term management is also outlined.
