CorHealth Ontario & Ontario Stroke System
In 2008, the Ontario Stroke Network (OSN) was created to provide provincial leadership and coordination for the Ontario Stroke System by recommending, implementing and evaluating province-wide goals and standards for the continuum of stroke care. In April, 2016 the OSN began the journey of merging with the Cardiac Care Network of Ontario
As of June 22, 2017, CorHealth Ontario was formed by the merger of the Cardiac Care Network of Ontario and the Ontario Stroke Network, with an expanded mandate spanning cardiac, stroke and vascular services through the entire course of care. CorHealth Ontario provides a strategic understanding of the care needs of both patients and providers, as the basis for clinical quality improvement, provincial planning, resource allocation and measurement performance of quality and outcomes.
Ontario Stroke System
The Ontario Stroke System (OSS) is a collaborative system of provider organizations and partners that aims to coordinate and improve stroke prevention and care across the entire continuum. Its leadership is provided by the CorHealth, and it is funded by the Ministry of Health and Long-Term Care and the Heart and Stroke Foundation of Ontario. The Southwestern Ontario Stroke Network is a part of the Ontario Stroke System.
